Form 13978: Projected Operations VITA Grant Application

Form 13978 is the Projected Operations VITA Grant Application. The form says it is to be used by VITA Grant applicants and should accompany the VITA Grant Application, with estimates expected for the next filing season. It is completed by the applying organization, not by taxpayers, and a note on page 1 says it is to be completed when applying for entry into the program.

Page 1 covers the period of performance from October 1 to September 30, the number of federal returns to be prepared, the number of sites by type, volunteers and volunteer hours by position, and returns for each underserved population. Page 2 is a table of existing and proposed sites with the EFIN and SIDN for each. Page 3 holds the line-by-line instructions. The form notes that at least 90% of the total number of federal returns prepared must meet the low income threshold.
