Contact Information for VITA and TCE Grant Programs
Form 14335, Contact Information for VITA and TCE Grant Programs, is a two-page IRS form for organizations that receive a Volunteer Income Tax Assistance (VITA) or Tax Counseling for the Elderly (TCE) grant. The instructions on the form say to list those individuals that the IRS may contact with questions concerning the grant.
The form starts with the recipient name, Federal Award Identifier, address and days and hours of operation. It then has blocks for a primary contact, an alternate contact, the Authorized Organization Representative, a federal tax matters contact, a financial/PMS contact and up to three other contacts, each with name, title, phone, extension, cell phone and email. Most blocks ask whether the IRS may share that contact's information with others involved in the VITA or TCE program.
The instructions and definitions are printed on page 2. You can fill out Form 14335 free in your browser on this page and download it without an account.

Contact fields for the federal tax matters contact and the financial/PMS contact. These two blocks have no sharing question.
Three blocks, one on page 1 and two on page 2, each with a space to explain the role and a sharing question.
Instructions, six role definitions, and the Privacy and Paperwork Reduction Act Notice on page 2.
Enter the recipient name, Federal Award Identifier, address, and days and hours of operation.
Enter one primary contact with name, title, phone, extension, cell phone and email, and answer the sharing question.
Enter at least one alternate contact with the same details and answer the sharing question.
Enter the Authorized Organization Representative, the federal tax matters contact and the financial/PMS contact.
For each other contact, explain the role in the space provided, as the definitions ask, and enter the contact details.
Check Yes or No on each sharing question. The instructions say the IRS is occasionally asked to share contact information with other grant recipients and non-grant recipients involved or interested in the VITA or TCE program.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form prints no mailing address or deadline. It is completed by the grant recipient for the IRS.
The instructions ask the recipient to indicate whether the IRS may share each contact's information with other grant recipients and non-grant recipients.
The definitions say this person is only contacted when questions arise concerning the organization's filing of its federal returns or paying of federal taxes, penalties or interest.
The notice gives OMB Control Number 1545-2222, an estimated 30 minutes, and says the response is voluntary.
