Transfer Order Surplus Personal Property
Standard Form 123 (SF 123), Transfer Order Surplus Personal Property, is a U.S. General Services Administration (GSA) form. Its footer prints STANDARD FORM 123 (REV. 6/2025) and Prescribed by GSA FMR (41 CFR) 102-37, with OMB Control Number 3090-0014. GSA hosts it in its forms library.
The form is a transfer order for surplus personal property going to donation. Its order types are State Agency, DOD (SEA) for a service educational activity, and FAA for a public airport. It names the holding agency and the property location, lists the property in block 12, and has signature blocks for the transferee, a Department of Defense or Federal Aviation Administration determining officer and a GSA approving officer. Pages 2 and 3 print the certifications, agreements and assurances the transferee makes.
GSA estimates 8 minutes to read the instructions, gather the facts and answer the questions. You can fill it out free in your browser on this page and download the result without an account.

The DOD or FAA determining officer and the GSA approving officer certify that the administrative actions for the order are being taken, and sign and date.

The PDF prints no filing address. Block 7 is addressed to GSA (General Services Administration), and block 14e is signed by a GSA approving officer.
The form says property approved by GSA will be held by the holding agency for a period not to exceed 42 calendar days from the surplus release date pending receipt of pickup or shipping instructions, and then released to the donee.
