Report of Excess Personal Property
Standard Form 120 (SF 120), Report of Excess Personal Property, is a one-page report from a reporting agency, named in block 7, to the agency named in block 5. The footer prints GENERAL SERVICES ADMINISTRATION and says the form is prescribed by (41 CFR) FMR 102-36.215, and GSA hosts a copy in its forms library. GSA's page lists the same citation as its authority or regulation.
The top blocks identify the report and its type, the agencies and contacts involved, where purchase orders or disposal instructions go, the location of the property and whether reimbursement is required. Below them, the excess property list has columns (a) to (i) for each item: FSC group, description, item number, condition, unit, number of units, acquisition cost per unit and in total, and fair value %.
The footer says to use Standard Form 120A for continuation sheets. You can fill it out free in your browser on this page and download the result without an account.
