Standard Form 122 (SF 122): Transfer Order Excess Personal Property

Standard Form 122 (SF 122), Transfer Order Excess Personal Property, is a one-page order addressed in block 3 to the General Services Administration by the ordering agency named in block 4. The header prints GENERAL SERVICES ADMINISTRATION, September 2014, and FMR (41 CFR) 102-36.125, 102-36.145 and 102-36.160, and GSA hosts a copy in its forms library. GSA's page lists the same three citations as its authority or regulation.

The form asks for the holding agency, where the property is and where it ships, shipping instructions, the ordering agency's approval, the appropriation and allotment, the Government B/L number and a table of the property ordered with unit, quantity and acquisition cost. Block 14 is for GSA approval, and a strip at the bottom is marked for GSA use only.

The form asks for a ZIP Code in each block marked with an asterisk. You can fill it out free in your browser on this page and download the result without an account.
