GSA Form 1039: Record of Property Found and Attempts to Contact Owner

GSA Form 1039, Record of Property Found and Attempts to Contact Owner, is a General Services Administration form, GSA FORM 1039 (REV. 1/77), hosted in GSA's forms library. It is a two-sided record with the field office, month and year at the top.

The front lists each item of found property by tag number: the dates it was found and turned in, the officer who received it, a description, where it was found and who found it, and its disposition, with the date, the signature and address of the receiver or an excess property report number, and the initials of the authorized GSA-PBS official who released it. The back, Record of Attempts to Contact Owner of Found Property, logs each attempt by tag number, date, owner's name and address, telephone number if known, telephone or letter, and the officer's name. The form prints no instructions, OMB number or privacy statement.
