Form B 107: Statement of Financial Affairs for Individuals Filing for Bankruptcy

Form B 107, Statement of Financial Affairs for Individuals Filing for Bankruptcy, is the Official Bankruptcy Form an individual debtor files with the United States Bankruptcy Court to give a summary of their financial history over certain periods of time before the bankruptcy. The U.S. Courts list it under Individual Debtors. The current version is dated 04/25, and the U.S. Courts page lists it as effective on April 1, 2022 and updated on April 1, 2025. The page also says Official Bankruptcy Forms are approved by the Judicial Conference and must be used under Bankruptcy Rule 9009.

The 12-page form has 28 questions in 12 parts: marital status and past addresses, income, payments to creditors and insiders, lawsuits and repossessions, gifts, losses, transfers, financial accounts and storage units, property held for someone else, environmental information and business connections, then a signature under penalty of perjury. It is derived from former Official Form 7.

This page explains the form in plain words and is not legal advice. You can fill it out free in your browser on this page and download the result without an account.
