Bankruptcy Petition Preparer’s Notice, Declaration and Signature
Form B 119, Bankruptcy Petition Preparer's Notice, Declaration and Signature, is the Official Bankruptcy Form that a bankruptcy petition preparer, as defined in 11 U.S.C. § 110, fills out when they help prepare documents filed in an individual's bankruptcy case. The U.S. Courts list it under Individual Debtors. The current version is dated 12/15, and the U.S. Courts page lists it as effective on December 1, 2015. The page also says Official Bankruptcy Forms are approved by the Judicial Conference and must be used under Bankruptcy Rule 9009.
The 2-page form has a notice to the debtor, which the debtor signs to acknowledge receipt, and a declaration that the preparer signs under penalty of perjury with a checklist of the documents prepared. It is derived from former Official Form 19.
