Request for Taxpayer Advocate Service Assistance (and Application for Taxpayer Assistance Order)
Form 911, Request for Taxpayer Advocate Service Assistance (and Application for Taxpayer Assistance Order), asks the Taxpayer Advocate Service (TAS) for help with a federal tax issue. The form describes TAS as an independent organization within the IRS that helps taxpayers resolve federal tax issues and protects taxpayers' rights, and says its assistance is always free.
The form says TAS may be able to help if a federal tax problem is causing a financial difficulty, if the issue could not be resolved with the IRS, or if an IRS system, process or procedure is not working as it should. Section I holds the taxpayer's details, the tax form and period, the issue, the relief requested and signatures. Section II is for an authorized third party under Form 2848 or Form 8821. The instructions are on pages 3 and 4.
You can fill out Form 911 free in your browser on this page and download the PDF without an account.
Taxpayer and spouse names and identifying numbers, current street address, daytime phone, email address and fax number.
Preferred way to receive updates (email, letter or phone call), consent to leave confidential details on voicemail, a point of contact, and preferred language or TTY/TDD needs.
The tax form number, such as 1040, 941 or 720, and the tax year or period.
A description of the tax issue and any difficulties it is creating, and the relief or assistance requested.
Signature of the taxpayer or corporate officer with title and date, and the spouse's signature for a joint request, under a statement authorizing TAS to contact third parties.
Part A for a Form 2848 power of attorney and Part B for a Form 8821 authorization, each with name, CAF number, address, phone, fax and the representative's signature.
Enter the name as shown on the tax return and the SSN, ITIN or EIN, a spouse's name and number if the request relates to a joint return, and the current mailing address.
Enter phone and fax. The form says an email address on line 5 is consent to encrypted email, so leave it blank to get no emails from TAS. Check 7b only to allow confidential details on voicemail.
Enter the tax form, for example Form 1040 for an individual income tax issue, and the period: the calendar or fiscal year for income tax, or the calendar quarter for employment tax.
Describe the issue and the actions the IRS has taken or not taken. For an IRS delay of more than 30 days, give the date of first contact. Then describe the action requested and attach supporting documents.
Sign and date. The instructions say signing does not suspend any applicable periods, such as for petitioning the Tax Court or requesting a Collection Due Process hearing.
Complete it only if a representative acts on the taxpayer's behalf. Leave unknown fields blank and include a copy of Form 2848 or 8821 if available.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form lists [email protected]. It says emailed forms and attachments are not encrypted, and sending one is consent to email as a way to communicate about the request.

The form says not to submit multiple Form 911s for the same issue, as this may cause delays.
