Schedule 6 (Form 8849), Other Claims, is attached to Form 8849, Claim for Refund of Excise Taxes. The instructions say it is used for claims not reportable on Schedules 1, 2, 3, 5 and 8, including refunds of excise taxes reported on Form 720, Form 2290, Form 730 and Form 11-C.
The schedule is a single page. It asks for the name and EIN or SSN as shown on Form 8849, the total refund, the earliest and latest dates of the events in the claim, up to five lines of tax, refund amount and CRN, and a space to explain each tax claimed. The schedule is the August 2013 revision. Its instructions were revised in June 2026 and add refund claims for the remittance transfer tax on canceled or expired remittance transfers.
You can fill out the PDF free in your browser on this page and download it without an account.
