Lines 1a to 2b: taxpayer names and numbers
Name and SSN, ITIN or EIN of the first taxpayer, and the spouse's if it was a joint return.
4506-T
Request for Transcript of Tax Return
Form 4506-T asks the IRS for tax return information in the form of a transcript. A transcript is a summary, not a photocopy of your return. If you need a copy of a filed return with its attachments, the IRS says to use Form 4506 instead.
People use this form to get proof of income for a loan, to check what the IRS has on file, or to show that no return was filed for a year. Taxpayers who use a fiscal tax year must file Form 4506-T to request a return transcript.
The IRS also offers faster routes: Get Transcript online at IRS.gov, or the automated phone line at 1-800-908-9946. You can fill out Form 4506-T on this page, in your browser, with no account.

Ask for verification of nonfiling or for a wage and income transcript, if that is what you need.

Enter the end date, for example 12/31/2018 for a calendar year 2018 Form 1040. Enter each quarter for quarterly returns.

The form says not to sign until every applicable line is completed, and the request may be rejected if it is incomplete or illegible.
