Identity Theft Victim’s Request for Copy of Fraudulent Tax Return
Form 4506-F, Identity Theft Victim's Request for Copy of Fraudulent Tax Return, is for a person whose name and Social Security number (SSN) were used to file a fraudulent tax return. The instructions say victims of identity theft use it to request a tax return transcript of a fraudulent return filed using their name and SSN as the primary or secondary taxpayer.
The form says that, due to federal privacy laws, the victim's name and SSN have to be listed as the primary or secondary taxpayer on the fraudulent return, and that the IRS cannot disclose return information to any person listed only as a dependent. The instructions add that some information on the transcript will be masked or partially masked, but there will be enough data to determine how the taxpayer's personal information was used.
The completed form goes to the IRS by mail or by fax. You can fill out Form 4506-F free in your browser on this page and download it without an account.
Line 1 is the SSN used on the fraudulent return and line 2 the name used on it. Lines 3 and 3a hold the SSN owner's current mailing address.
The SSN owner's complete address shown on the last return, if it is different from the current mailing address.
Line 5 holds four relationship boxes. Line 6 holds the requestor's SSN or TIN, name and mailing address when different from Step 1, and line 6a a representative CAF number.
Line 7 lists the tax year or years of the individual income tax forms being requested.
A declaration, the requestor's signature, printed name, date and daytime telephone number, and a note about documents showing authority.
Enter the SSN used on the fraudulent return and the name of the taxpayer used on that return.
Enter the SSN owner's current mailing address, including a P.O. box if one is used, then city, state and ZIP code. Line 4 takes the complete address on the last return filed if it differs. The instructions say Form 8822 is filed if the addresses differ and the address has not been changed with the IRS.
Check the box that describes the relationship to the taxpayer in Step 1. A different requestor enters an SSN or appropriate TIN, name and mailing address, and a CAF number if the authority is on file with the IRS.
Enter the tax year or years requested. The instructions say copies of fraudulent returns can be requested for the current tax year and previous six tax years.
Complete all applicable lines before signing, then sign, print the name, date it and add a daytime phone number. The form says a request may be rejected if the form is incomplete or illegible.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.

The instructions give the toll-free fax number 855-807-5720 and ask for a cover sheet marked 'Confidential'.
To help avoid delays, the instructions say to choose one method of submitting the form, either by mail or by fax, not both.
If the requestor's authority is not on file with the IRS, the instructions say the form is accompanied by documents such as Form 2848, Form 8821 or a court order, except when a parent or legal guardian requests a minor child's information.
The instructions say a request missing any required item will be rejected as incomplete and returned to secure the missing information.
Line 5 lists four kinds of requestor. The instructions say the form is signed and dated by the taxpayer listed on line 2 or the requestor listed on line 6.
