VA Form 26-8629 is the U.S. Department of Veterans Affairs Manufactured Home Loan Claim Under Loan Guaranty. VA's form page lists it under housing assistance. The form's heading says it covers the manufactured home unit only, under Section 3720, Chapter 37, Title 38 U.S.C. Its respondent burden notice says VA needs the information to determine the amount payable under the loan guaranty.

In the certification, the undersigned certifies that it is the owner of the loan and that the Certificate of Guaranty or Endorsement of Guaranty will be surrendered or canceled in accordance with 38 CFR 36.4218 upon full payment of the claim.

The form is two pages. Page 1 holds the loan and obligor details, the status of the account and the final accounting on liquidation of the security.
