Request for Verification of Deposit
VA Form 26-8497a is the U.S. Department of Veterans Affairs Request for Verification of Deposit. VA's form page lists it under housing assistance. The respondent burden notice says VA needs the information to help determine a veteran's qualifications for a VA-guaranteed loan, and the Privacy Act notice says information may be disclosed to depository institutions so they can provide information on assets for credit underwriting.
The form is one page. A lender or Local Processing Agency fills in Part I, the request, and has the applicant sign it, then sends it directly to the bank or other depository. The depository completes Part II with the applicant's deposit accounts and outstanding loans and returns it directly to the lender or agency.
