VA Form 26-6807: Financial Statement

VA Form 26-6807 is the U.S. Department of Veterans Affairs Financial Statement. Its Privacy Act notice says the information is used to service your loan and to evaluate your application for release of liability and, if applicable, substitution of entitlement. VA's form page lists it under housing assistance.

The form is four pages with seven sections: general information, employment and financial status, credit references, real estate owned, additional data, remarks and certifications. Page 4 holds the Privacy Act and respondent burden notices, a Right to Financial Privacy Act notice and the government monitoring information. The top of page 1 has a file number and a loan number.

The form says to type or print all entries in ink, and that a co-borrower or co-applicant who is not the spouse of the borrower/applicant should complete a separate financial statement. You can fill out the PDF free in your browser on this page and download the result without an account.
