Loan Service Report
VA Form 26-6808 is the U.S. Department of Veterans Affairs Loan Service Report. It records an interview with a borrower, held in the field, by phone, in the office or by email, about the borrower's finances, the property and a delinquency on the loan. The respondent burden note says VA needs the information to provide financial counseling under Title 38 USC 3732(a)(4). VA's form page lists it under housing assistance.
The form is two pages. Page 1 holds the interview details, Section I financial information with monthly expenses, income, discretionary income and assets, the borrower's explanation of the delinquency, and Section II certifications for the borrower and spouse. Page 2 holds Section III property information, the representative's opinion of the cause of the delinquency, a proposed repayment schedule, recommendations and the representative's signature.
The Privacy Act notice says that without this information VA may be unable to provide financial counseling or assistance in dealing with your mortgage loan holder. You can fill out the PDF free in your browser on this page and download the result without an account.
