VA Form 26-8844 is the U.S. Department of Veterans Affairs Financial Counseling Statement. It covers the homeowner's and spouse's employment, monthly income and debts, the reason for a delinquency, a monthly budget and a net worth statement.
The form is two pages. Its Privacy Act notice says the obligation to respond is voluntary, but without this information VA may be unable to provide financial counseling or assistance in dealing with your mortgage loan holder. It has signature lines for the borrower or applicant, the spouse and a representative, and estimates a respondent burden of 45 minutes.
