VA Form 26-8497: Request for Verification of Employment

VA Form 26-8497 is the U.S. Department of Veterans Affairs Request for Verification of Employment. VA's form page lists it under housing assistance. The form carries the names of both the Department of Veterans Affairs and the U.S. Department of Agriculture, and its footer also reads USDA Form 410-5. The respondent burden notice says VA needs the information to help determine a veteran's qualifications for a VA-guaranteed loan.

The form is one page. The lender or Local Processing Agency fills in the request in Part I and has the applicant sign it, then sends it straight to the employer. The employer answers either Part II, for present employment, or Part III, for previous employment, and signs the certification in Part IV before returning it straight to the lender or agency.
