Notice of Appeal of Decision Under Sections 210 or 245A of the Immigration and Nationality Act
Form I-694, Notice of Appeal of Decision Under Sections 210 or 245A of the Immigration and Nationality Act, is used to notify USCIS that a person who was denied permanent residence, temporary residence or a waiver of grounds of inadmissibility under the amnesty program provisions of the Immigration Reform and Control Act of 1986 (IRCA) is appealing the decision. The USCIS page says the appeal goes to the USCIS Administrative Appeals Office.
Part 2 of the form ties the appeal to the denied application: Form I-698 (permanent residence), Form I-687 (temporary residence) or Form I-690 (waiver of grounds of inadmissibility). Part 3 records whether a written brief is attached.
The instructions say the appeal is filed with the USCIS office that decided the case, as shown on the Notice of Denial, and that USCIS has to receive it within 30 calendar days of the date on that notice. They also say not to file the appeal at the Administrative Appeals Office.

A stamped or typewritten name is not accepted, and the USCIS page says an unsigned form is rejected. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page says to mail the form to the address given on the Notice of Denial. The instructions say to file it with the USCIS office that decided the case, not at the Administrative Appeals Office.
The instructions say USCIS has to receive the appeal within 30 calendar days of the date on the Notice of Denial, counting from 3 days after the notice is mailed, with no extensions.
The fee is on the USCIS Fee Schedule page (Form G-1055), which can change. The instructions say an appeal that is unsigned or without the correct fee is rejected as deficient and can be corrected and resubmitted.
A brief is not required. The instructions say it can go with the form or separately, and a separate brief has to reach USCIS within 30 calendar days of the appeal's receipt date.
A request goes in a letter with the form. The instructions say an oral argument is limited to 15 minutes unless more time is arranged in advance, and the appellant appears in person at a USCIS office if it is granted.
The USCIS page says Form G-1145 can be clipped to the first page to get an email or text message when a USCIS lockbox accepts the form.
