Application for Waiver of Grounds of Inadmissibility Under Sections 245A or 210 of the Immigration and Nationality Act
Form I-690, Application for Waiver of Grounds of Inadmissibility Under Sections 245A or 210 of the Immigration and Nationality Act, is used by an applicant for adjustment of status under INA section 210 (Special Agricultural Workers) or section 245A (Legalization, including LIFE Act Legalization) to ask USCIS to waive a ground of inadmissibility.
The instructions say USCIS may waive certain grounds at its discretion for humanitarian purposes, family unity or the public interest. Part 2 of the form names the primary application the waiver is for: permanent residence (Form I-698 or LIFE Act Form I-485) or temporary residence (Form I-687 or Form I-700).
The instructions also list grounds that do not apply to these applicants and grounds that cannot be waived, such as multiple criminal convictions and security grounds. An applicant with a Class A tuberculosis condition also completes Supplement 1, which the USCIS page offers as a separate download.

Legal name, mailing and physical address, city and country of birth, date of birth, A-Number, USCIS online account number and Social Security number, if any.
Whether the waiver is for permanent residence (Form I-698 or LIFE Act Form I-485) or temporary residence (Form I-687 or Form I-700), with the filing date and receipt number.
Check boxes for each INA section 212(a) ground the waiver covers, plus space to list the specific reasons the applicant is inadmissible.
Up to four immediate relatives in the United States (parents, spouse and children) with their immigration status, and a description of the family unity, humanitarian or public interest reasons for the waiver.
The applicant's contact information, certification and signature, then sections for an interpreter and for a preparer, if other than the applicant.

Extra space for longer answers, with the page, part and item number each answer refers to.
The USCIS page lists edition 04/01/24 and says all pages need to be from the same edition, with the edition date and page numbers visible on a printed copy. Type or print legibly in black ink.
The USCIS page says it rejects the form if the Family Name or Mailing Address in Part 1 is missing. Write N/A where a question does not apply and None for a zero.
Select permanent or temporary residence, then enter the date the primary application was filed and its receipt number.
Check every INA section that applies in Item 4, list the reasons in Item 5, and describe the humanitarian, family unity or public interest reasons in Item 7. Use Part 6 for more space.
Supplement 1, Applicants with a Class A Tuberculosis Condition, is a separate download on the USCIS page and goes with Form I-690 in that case.
The instructions say a stamped or typewritten name is not accepted, and a parent or legal guardian may sign for an applicant under 14. This page lets you fill the form online free, then print it for signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The USCIS page lists the USCIS Dallas Lockbox, with a P.O. Box for U.S. Postal Service mail and a Lewisville, Texas street address for FedEx, UPS and DHL deliveries. The instructions point to the Form I-690 page for current filing information.
The fee is on the USCIS Fee Schedule page (Form G-1055), which can change. Check it on the day you file.
The instructions describe evidence that a waiver would serve humanitarian purposes, family unity or the public interest, and that approval is warranted as a matter of discretion, with favorable factors outweighing unfavorable ones.
For a drug abuse or addiction waiver, the instructions list a copy of the medical examination report (Form I-693 or Form DS-2054), related medical documents and other relevant reports. For a vaccination waiver, the USCIS checklist lists evidence of a sincere religious or moral objection.
Send legible photocopies unless an original is requested. Any foreign-language document needs a full English translation with the translator's certification.
USCIS may schedule a biometrics appointment or an interview. The instructions say a United States address is needed to file, and USCIS sends its decision in writing.

The instructions say the applicant and the treating physician or medical facility complete Supplement 1, and a state health department official signs its Part 5. For an applicant outside the United States, a relative in the United States completes this process.
The instructions list other grounds that may be waived, such as a single offense of simple possession of 30 grams or less of marijuana, misrepresentation, previous removal and unlawful presence.
The instructions say entry without labor certification, presence without admission or parole, and immigrant documentation requirements do not apply to SAW or Legalization applicants. Some vaccination cases get a blanket waiver instead.
The instructions list crimes involving moral turpitude, controlled substance violations other than the marijuana exception, multiple convictions, trafficking, security grounds and public charge, except for Form I-687 applicants for temporary residence.
