Standard Form 30 (SF 30): Amendment of Solicitation/Modification of Contract

Standard Form 30 (SF 30), Amendment of Solicitation/Modification of Contract, is the form an issuing office uses to amend a solicitation or to modify a contract or order. The form says it is prescribed by GSA under FAR (48 CFR) 53.243. This copy is Rev. 11/2016, and the form says the previous edition is unusable. GSA hosts it in its forms library.

Item 11 applies only to amendments of solicitations and Item 13 applies only to modifications of contracts and orders. Item 14 describes the change, organized by Uniform Contract Format (UCF) section headings. The contractor or offeror signs in Items 15A to 15C when required, and the contracting officer signs in Items 16A to 16C. The back page has instructions for the items that are not self-explanatory.

Where it goes and what the form says

Who it goes to

The PDF gives no filing address. The issuing office named in Item 6 issues it, and when Item 13E requires a signature, the contractor returns copies to the issuing office.

Acknowledging an amendment

Item 11 lists three methods: completing Items 8 and 15 and returning copies, acknowledging receipt on each copy of the offer, or a separate letter or electronic communication. The form says late acknowledgment may result in rejection of the offer.

Changing an offer already submitted

The form says a change may be made by letter or electronic communication that references the solicitation and the amendment and is received before the opening hour and date specified.

Terms not changed

The form says that, except as provided in it, all terms and conditions of the document in Item 9A or 10A, as changed before, remain unchanged and in full force and effect.

Who fills out SF 30?

  • The issuing office

Item 6 holds the name, address and code of the issuing office, and Item 7 the administering office if it is different.

  • The contracting officer

Signs Items 16A to 16C for the United States of America. The form says this signature is not required on solicitation amendments and is normally affixed last on supplemental agreements.

  • The contractor

Item 13E shows whether the contractor is or is not required to sign the document and return copies to the issuing office. The contractor's signer gives a name and title in Item 15A.

  • Offerors on a solicitation

Item 11 says offers must acknowledge receipt of an amendment before the hour and date specified in the solicitation, by one of three methods the form lists.

The issuing office prepares the form. Item 13E says whether the contractor signs it.
