Solicitation/Contract/Order for Commercial Products and Commercial Services
Standard Form 1449 (SF 1449) is the Solicitation/Contract/Order for Commercial Products and Commercial Services. Its footer says it is prescribed by GSA under FAR (48 CFR) 53.212, and GSA hosts it in its forms library. This edition is Rev. 11/2021, and the form says the previous edition is not usable.
The front holds the solicitation, the offer and the award. Block 14 marks the method of solicitation as request for quote (RFQ), invitation for bid (IFB) or request for proposal (RFP). A note at the top says the offeror completes blocks 12, 17, 23, 24 and 30, and the contracting officer signs the award in block 31.
The back continues the schedule and adds receipt, inspection and acceptance by an authorized government representative, plus payment and receiving records. You can fill it out free in your browser on this page and download the result without an account.

You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF prints no address. Block 28 says signed copies go to the issuing office, and block 9 names the office that issued it.
Block 18b says to submit invoices to the address shown in block 18a unless the block below is checked, which points to an addendum.
Block 11 says delivery is free on board (FOB) destination unless the block is marked, with a box to see the schedule.
The form says it is authorized for local reproduction and that the previous edition is not usable. GSA's page lists the authority as FAR (48 CFR) 53.212.
