Optional Form 347 (OF 347): Order for Supplies or Services

Optional Form 347 (OF 347), Order for Supplies or Services, is an order signed for the United States of America by a contracting/ordering officer. The footer reads OPTIONAL FORM 347 (REV. 2/2012) and Prescribed by GSA/FAR 48 CFR 53.213(f), and GSA's page lists the authority as FAR (48 CFR) 53.213(f). GSA hosts a copy in its forms library, where it is listed as Order for Supplies and Services.

Item 8 marks the order as a purchase or as a delivery order under an existing contract. The front holds the issuing office, the contractor, shipping and accounting data, business classification, a schedule of items with quantities and prices, where to mail the invoice and the officer's signature. The back holds supplemental invoicing information, a receiving report and a report of rejections. The form says it is authorized for local reproduction and that the previous edition is not usable.

Invoicing and what the form says

The order as an invoice

The form says the contractor may use the order, or a copy, as its invoice instead of a separate invoice, if a signed and dated statement requesting payment of the amount and saying no other invoice will be submitted is on or attached to it.

A separate invoice

The form says a separate invoice gives the contract number (if any), order number, item numbers, description, sizes, quantities, unit prices and extended totals, with prepaid shipping costs as a separate item.

Shipping costs

The form says that where shipping costs exceed $10 (except for parcel post), the billing must be supported by a bill of lading or receipt.

Consolidated billing

The form says that when several orders are invoiced to an ordering activity during the same billing period, consolidated periodic billings are encouraged.

Where invoices go

The PDF prints no fixed address. Item 21 names the office the invoice is mailed to, and item 5 names the issuing office for correspondence.
