Order for Supplies and Services Schedule - Continuation
Optional Form 348 (OF 348), Order for Supplies or Services Schedule - Continuation, is a one-page continuation sheet for the schedule of an order for supplies or services. The footer reads OPTIONAL FORM 348 (REV. 4/2006) and Prescribed by GSA-FAR (48 CFR) 53.213(f). GSA hosts a copy in its forms library, where it is listed as Order for Supplies and Services Schedule - Continuation.
The sheet repeats the date of order, contract number and order number, then gives more lines for the schedule: item number, supplies or services, quantity ordered, unit, unit price, amount and quantity accepted. Its total is carried forward to the first page, item 17h. On OF 347, Order for Supplies or Services, item 17(h) is the total of continuation pages.
You can fill it out free in your browser on this page and download the result without an account.
