Standard Form 294 (SF 294): Subcontracting Report for Individual Contracts

Standard Form 294 (SF 294), Subcontracting Report for Individual Contracts, collects subcontract award data from prime contractors and subcontractors under a subcontracting plan. The form says it is prescribed by GSA under FAR (48 CFR) 53.219, and it carries OMB Control Number 9000-0007 with an expiration date of 4/30/2028. GSA hosts it in its forms library.

The form says the report is required for each contract containing a subcontracting plan and goes to the administrative contracting officer (ACO), or the contracting officer if no ACO is assigned, semi-annually for the periods ended March 31st and September 30th, with a separate report at contract completion. It also says the report is not required for small businesses.

The form estimates 2 hours to read the instructions, gather the facts and answer the questions. You can fill it out free in your browser on this page and download the result without an account.

How to complete SF 294

  1. 1

    Identify the establishment

Block 2 takes the unique entity identifier for the specific contractor establishment. If none matches the name and address in Block 1, the form says to contact the entity designated at SAM.

  2. 2

Pick the period and report type

Check one period in Block 4. The data covers activity from the inception of the contract through that date. The form says Final is checked only when the contract is completed, and Revised changes an earlier report for the same period.

  3. 3

    Enter goals from the plan

Under Current Goal, enter the dollar and percent goals from the subcontracting plan approved for the contract. If modifications revised the original goals, the form says to enter the original goals in Block 19.

  4. 4

    Report actual cumulative awards

Enter achievements since the inception of the contract, on the same basis as the goals. The form says to report only awards to immediate subcontractors and to leave out purchases from affiliates.

  5. 5

    Explain shortfalls in Block 19

The form asks for a short narrative when accomplishments fall below a straight-line projection of the goals, or, on a final report, when any of the six goals were not met.

  6. 6

Send it to the contracting officer

The original goes to the contracting officer at the agency or contractor in Block 8, with a DCMA copy for DOD contracts.
