Standard Form 2803 (SF 2803): Application to Make Deposit or Redeposit (CSRS)

Standard Form 2803 (SF 2803), Application to Make Deposit or Redeposit, Civil Service Retirement System, is a U.S. Office of Personnel Management (OPM) form, Revised June 2013, under the CSRS/FERS Handbook for Personnel and Payroll Offices, with OMB No. 3206-0134. GSA hosts a copy in its forms library. The form says it is for people covered by the Civil Service Retirement System (CSRS) who want to pay a deposit or a redeposit for civilian service, and not for people covered by the Federal Employees Retirement System (FERS).

The form describes a deposit as a payment to the retirement fund for civilian service during which no retirement deductions were withheld, and a redeposit as a payment for service whose deductions were withheld and later refunded. The package holds Part A for the applicant, Part B for the employing agency, an information sheet on deposits, redeposits and interest, and SF 2803A, Application to Pay Military Deposit for military service performed after December 31, 1956.

  2. 2

List the service to pay for

  5. 5

Sign item 13 and add an email address, a telephone number where you can be reached during the day, and the date.

  6. 6
