Standard Form 2801 (SF 2801): Application for Immediate Retirement (CSRS)

Standard Form 2801 (SF 2801), Application for Immediate Retirement, Civil Service Retirement System (CSRS), is a U.S. Office of Personnel Management (OPM) form, Revised June 2013, under the CSRS/FERS Handbook for Personnel and Payroll Offices. GSA hosts a copy in its forms library. The form says it is for a current Federal or Postal Service employee covered by CSRS who wishes to apply for retirement with an immediate annuity, one commencing within one month after the date of separation on which title to annuity is based.

The package holds instructions and retirement information, the application (Sections A to I and an applicant's checklist), Schedules A, B and C, SF 2801-2 Spouse's Consent to Survivor Election, SF 2801-1 Certified Summary of Federal Service, and Schedule D, the agency checklist. It refers to the pamphlet SF 2801A for more information.

The form says not to use it to apply for a deferred annuity, and that employees covered by the Federal Employees Retirement System use SF 3107. You can fill it out free in your browser on this page and download the result without an account.
