Application to Make Voluntary Contributions - CSRS
Standard Form 2804 (SF 2804), Application to Make Voluntary Contributions, Civil Service Retirement System, is a U.S. Office of Personnel Management (OPM) form, Revised July 2006, under the CSRS and FERS Handbook for Personnel and Payroll Offices. GSA hosts a copy in its forms library. The footer says the July 1999 edition is usable and all other previous editions are unusable.
The information sheet describes voluntary contributions as optional payments made to the Civil Service Retirement and Disability Fund for the purchase of additional annuity at retirement. It says no monies are accepted until OPM approves the SF 2804, and that employees covered by or retiring under the Federal Employees Retirement System (FERS) are not eligible to make voluntary contributions.
The one-page application has questions 1 to 8b, the applicant's signature and an agency certification, followed by an information sheet to detach and keep. You can fill it out free in your browser on this page and download the result without an account.

The form says to type or print in ink and answer all the questions.
The form says that if the answer to question 6b, 7b or 8b is No, do not file this application, and points to the attached information.
The applicant signs, dates and gives a telephone number under a statement of employment in a CSRS position, or of applying for CSRS retirement, and of having read the attached information.
An employee submits the completed form to the agency personnel office, which completes the agency certification box and forwards the application to OPM.
The form says not to send any payment with the application; a payment sent with it will be returned. OPM sends instructions and an account number once the application is accepted.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
Employees: the agency personnel office, which forwards it to OPM. People already separated for retirement and waiting for approval: U.S. Office of Personnel Management, Retirement Operations Center, PO Box 45, Boyers, PA 16017-0045.
The form says each payment is in multiples of $25 (i.e., $50, $75, $100, etc), and that contributions cannot be deducted from salary.
The form says total contributions cannot be more than 10 percent of the total basic civilian salary received, and any amount found over the limit is refunded without interest.
The form says to ask your personnel office if employed, or, if a personnel office is not available, to contact OPM at the address given or call 1-888-767-6738 (TDD: 1-800-878-5707).
Title 5, U.S. Code, Chapter 83, Civil Service Retirement authorizes the collection. The form says failure to supply all of the requested information may delay or prevent approval of the application.
Active employees covered under the Civil Service Retirement System may make voluntary contributions once OPM approves the application. The applicant answers questions 1 to 8b and signs.
Retired employees whose applications for CSRS retirement are being adjudicated may also apply. The form says no agency certification is needed for someone already separated for retirement.
For an employee, the agency personnel office completes the certification that the applicant is an agency employee in a CSRS position who answered question 6a correctly, then forwards it to OPM.
The form says employees covered by or retiring under FERS are not eligible. An employee who transfers from CSRS to FERS may not add to an existing account, which continues to earn interest.
The information sheet says who may make voluntary contributions and who certifies the application.

No. The form says not to send any payment with the application, that a payment sent with it will be returned, and that OPM sends full instructions once the application is accepted.
It says that at retirement at age 55 or younger, with no survivor benefit elected, each $100 to your credit purchases $7 a year of additional annuity, rising by 20 cents for each full year over 55, for example $8 a year at age 60 and $10 a year at age 70. Cost-of-living adjustments are not applied to it.
The form says a refund of all, not just a part, of the voluntary contributions plus earned interest may be paid at any time before retiring and receiving additional annuity, and that the account is then closed.
