Inventory Verification Survey
Standard Form 1423 (SF 1423), Inventory Verification Survey, is a two-page survey form. The footer says it is prescribed by GSA under FAR (48 CFR) 53.245(c), the authority GSA's page also lists, and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1423 (REV. 10/2023), and the heading points to the Federal Acquisition Regulation at FAR 45.602-1(b)(1).
Section I names the offices the survey goes from and to, the contract number and type, the contractor or subcontractor, the schedules of inventory to be inspected and verified, and the plant clearance case number. Section II asks yes or no questions about the property on the inventory disposal schedules, and Section III asks about termination inventory when the requester marks it as required. A verifier then signs under a statement that the information is based on a physical verification of the inventory listed under Item 5.
The form prints no OMB number and no time estimate. You can fill it out free in your browser on this page and download the result without an account.

The date, the From and To addresses with ZIP Codes, the contract number and type, the contractor or subcontractor, and Item 5B for the plant clearance case number or document number.
A table of the schedules of inventory to be inspected and verified, with a reference number, start and end page numbers and an amount in dollars for each.

Items 6 to 16 ask yes or no whether the property is on hand in the quantities indicated, correctly described, protected, tagged, coded and classified, whether weights are about right, and whether items need demilitarization or special processing.

Items 17 to 22 ask whether work stopped promptly on receipt of the termination notice, whether quantities exceed what the terminated portion needed, about rejects, about inspection of completed articles and about work-in-process completion.
Requesting office remarks, the signature of the requester, and the verifier's name and title, signature and date.
Enter the date, the From and To addresses with ZIP Codes, the contract number and type, the contractor or subcontractor and the plant clearance case or document number.
In Item 5A, list each schedule's reference number, its start and end page numbers and its amount in dollars.
The requester checks one box saying completion of Section III, Termination Inventory, Is or Is Not Required.
Check Yes or No for each question. Item 12 also asks for an estimate of weight by basic material content if weights are not shown, and Item 17 asks for the date of the termination notice.
Item 23 says that where an answer is placed in a block containing an asterisk (*), detailed comments of the Verifier shall be included on a continuation sheet and identified by Section and Item Number.
The requester signs Item 24. The verifier enters a name and title, signs and dates Items 25 to 27.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address. Item 1 names the office it is from and Item 2 the office it goes to, each with a ZIP Code.
Under Inventory Verification, the form says the above information is based on a physical verification of inventory listed under Item 5.
The heading points to FAR 45.602-1(b)(1), and the footer says the form is prescribed by GSA-FAR (48 CFR) 53.245(c).
The footer says Authorized For Local Reproduction and Previous Edition Is Not Usable.
Item 1 names the office the survey is from, and Item 24 is the Signature of Requester. The requester also checks whether Section III is or is not required.
Item 2, To, takes the receiving office's address with ZIP Code. The form does not say what kind of office this is.
Items 25 to 27 take the verifier's name and title, signature and date, under the heading Inventory Verification.
Item 4 names the contractor or subcontractor whose inventory is surveyed. Item 14 asks whether the contractor made reasonable efforts to return the property, and Item 20 says to obtain from the contractor the estimated cost of reworking rejects.
The blocks printed on the form name these roles. The form has no signature block for the contractor.

No tool matches that search.
The footer says the form is prescribed by GSA-FAR (48 CFR) 53.245(c), the authority GSA's page also lists. GSA hosts it in its forms library.
The requester signs Item 24. The verifier signs Item 26, with a name and title in Item 25 and the date in Item 27. There is no contractor signature block.
Item 23 says that where the answer to any question is placed in a block containing an asterisk (*), detailed comments of the Verifier shall be included on a continuation sheet and identified by Section and Item Number.
The form leaves that to the requester, who checks one box saying completion of Section III, Termination Inventory, Is or Is Not Required.
The two pages print no OMB control number and no time estimate. The footer shows STANDARD FORM 1423 (REV. 10/2023) and says the previous edition is not usable.
