Consent of Surety and Increase of Penalty
Standard Form 1415 (SF 1415), Consent of Surety and Increase of Penalty, is a two-sided form tied to a modification of a federal contract. The footer says it is prescribed by GSA under FAR (48 CFR) 53.228(l), and GSA hosts it in its forms library. The edition printed on the form is STANDARD FORM 1415 (REV. 10/2023), with OMB Control Number 9000-0001, and the footer says the previous edition is not usable.
In Item 4 the Surety (Co-Sureties) consents to the foregoing contract modification and agrees that its bond or bonds apply and extend to the contract as modified or amended. The Principal and Surety (Co-Sureties) further agree that the penalty of the performance bond or bonds and of the payment bond or bonds is increased by the amounts entered. Items 5 to 7 then show each surety's increase in liability limit under each bond.
GSA's page says the form is intended for use by government contractors and contracting personnel for compliance with and management of financial security requirements in Federal Government contracts. You can fill it out free in your browser on this page and download the result without an account.
