OGE Form 450: Confidential Financial Disclosure Report, Executive Branch

OGE Form 450 is the Confidential Financial Disclosure Report for the Executive Branch. It is a U.S. Office of Government Ethics (OGE) form, dated 08/2024, issued under 5 CFR Part 2634, Subpart I, with OMB No. 3209-0006 and an expiration date of 08/31/2027. GSA hosts it in its forms library, and GSA's page links to the PDF in OGE's forms library.

The form says its purpose is to assist employees and their agencies in avoiding conflicts between official duties and private financial interests or affiliations. It is filed by employees in positions designated for filing, either as new entrants or as annual filers, and it is reviewed by agency officials.

After a cover page with Yes or No statements, the report has Part I Assets and Income, Part II Liabilities, Part III Outside Positions, Part IV Agreements or Arrangements and Part V Gifts and Travel Reimbursements, plus continuation pages and a page of examples. You can fill it out free in your browser on this page and download the result without an account.
