Optional Form 301B-M (OF 301B-M): Volunteer Group Sign-up Form (Mandarin)

Optional Form 301B-M (OF 301B-M) is the Mandarin edition of the Volunteer Service Agreement, Natural and Cultural Resources Volunteer Sign-up Form for Groups. It is printed in Chinese under the title "志愿者服务协议-自然和文化资源团体志愿者登记表", and its footer reads "Mandarin OF 301B". The footer names USDOI, USDA, USDOC and USDOD, and the burden statement names the U.S. Departments of the Interior, Agriculture, Defense and Commerce. GSA hosts a copy in its forms library.

The form says all volunteers who take part with an organized group on an episodic volunteer project with a federal land and water management agency sign up on it, unless they sign up under an individual Volunteer Service Agreement (OF 301A). Photo consent

A yes or no box for consent to being photographed and to the release of the volunteer's photographic image.

    Enter the project name, group name, agency and group leader, and the agreement number from box 21 of the group's OF 301A.

    Each volunteer enters their name, e-mail address, telephone number and month and year of birth on a separate row. The second page holds more rows.

    Answer the two statements

    Each volunteer checks yes or no for the health and physical condition statement and for the photo consent statement.

    Each volunteer signs their own row in the signature column.
