Volunteer Service Agreement - Natural and Cultural Resources
Optional Form 301A (OF 301A), Volunteer Service Agreement - Natural & Cultural Resources, is a two-page agreement between a volunteer, or a group leader, and a federal agency for a specific volunteer activity. The form's footer names the U.S. Department of the Interior (USDOI), the U.S. Department of Agriculture (USDA), the U.S. Department of Commerce (USDOC) and the U.S. Department of Defense (USDOD), and it carries OMB Control Number 1093-0006 with an expiration date printed as 10/31/2024. GSA hosts a copy in its forms library.
The volunteer or group leader fills in contact details, optional demographic information and an emergency contact. A government official completes a section naming the agency, the agreement number, any approved reimbursements and the service to be performed. A parent or guardian signs a consent for a volunteer under age 18, the volunteer or group leader signs an affirmation, and a government representative signs for the agency. A last section records when the agreement ends and the total hours completed.
