OF301A-S
Volunteer Service Agreement - Natural and Cultural Resources (Spanish)
Optional Form 301A-S (OF 301A-S) is the Spanish-language edition of the Volunteer Service Agreement, Natural and Cultural Resources. It is printed in Spanish under the title "Acuerdo de Servicio Voluntario - Recursos Naturales y Culturales", and its footer reads "Spanish OF 301A". The footer names USDOI, USDA, USDOC and USDOD, and the burden statement names the U.S. Departments of the Interior, Agriculture, Defense and Commerce. GSA hosts a copy in its forms library.
The agreement covers an individual volunteer or a group leader, with contact and emergency details, an optional demographic section, the service to be performed, parental authorization for volunteers under 18, the volunteer's statements and signature, and a government representative's signature. The form says a government official completes blocks 20 to 27, and a last section records when the agreement ends and the total hours completed.
It is printed as Rev. 10/2021 with OMB control number 1093-0006. The labels on this page are English summaries of the Spanish form. You can fill it out free in your browser on this page and download the result without an account.

Individual or group, group name, name of the volunteer or group leader, U.S. citizen or permanent resident (with visa type if applicable), address, date of birth, telephone and e-mail.
Ethnicity, race (one or more), military veteran or active duty status, and disability. The form says this helps it understand diversity and inclusion in the volunteer force.
Name, telephone, e-mail and address of the emergency contact.
Agency or office, agreement number, agency contact, approved reimbursements with type and percentage, project name, a description of the service, and a checklist of attachments and requirements.
For volunteers under 18: the parent or guardian's name, telephone, e-mail and address, an authorization statement, the youth's name, signature and date.
Five statements for the volunteer or group leader to check, their signature and date, the government representative's signature and date, and the termination date, total hours and signature.
The 46 numbered blocks fall into these groups.
Check one agreement type in block 1. A group leader also enters the group name in block 2.
Complete blocks 3 to 11 and 13 to 19. Block 12 on ethnicity, race, veteran status and disability is marked optional.
The form says a government official completes this section. Block 26 asks for a summary of the service, with details such as time commitment, schedule, vehicle and equipment use, skills and physical activity level.
A parent or guardian completes blocks 28 to 38, including the youth's name in block 36 and a signature and date in blocks 37 and 38.
The volunteer or group leader checks the statements in block 39, agrees to follow applicable safety guidelines and signs and dates blocks 40 and 41.
A government representative signs and dates blocks 42 and 43. When the agreement ends, blocks 44 to 46 record the end date, total hours completed and a government signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no address. A government official completes the agency section and a government representative signs. The burden statement says persons with disabilities who need alternative means of communication should contact the volunteer program they are applying to.
The form says the agency provides the materials, equipment and facilities available and needed for the service, and treats the volunteer as a federal employee only for tort claims, civil liability and injury compensation, to the extent the volunteer group does not cover them.
The form says completing it is voluntary, but not providing the information will prevent participation. It estimates an average of 15 minutes per response under OMB control number 1093-0006.
Collection and use are covered by Privacy Act systems INTERIOR/DOI-05 and OPM/GOVT-1 under 5 USC 552a, and the form says the information is used to identify people interested in the volunteer program and to manage it.
For a Section 508-related complaint, the form gives the DOI Section 508 Program at [email protected] or (202) 208-1530.
