OF301A-M
Volunteer Service Agreement - Natural and Cultural Resources (Mandarin)
Optional Form 301A-M (OF 301A-M), 志愿者服务协议--自然与文化资源, is the Mandarin Chinese version of the volunteer service agreement. GSA's page lists it as Volunteer Service Agreement - Natural and Cultural Resources (Mandarin), and the form's footer reads OF301A-M (Mandarin OF 301A) (REV. 10/2021). The footer names USDOI, USDA, USDOC and USDOD, and the header prints OMB control number 1093-0006 with a date of 10/31/2024 (终止日期). GSA hosts a copy in its forms library.
The form, written in Chinese, is an agreement between a volunteer or group leader and a federal agency for a specific volunteer activity. It has the same 46 numbered items as OF 301A: the volunteer's details, optional demographic information, an emergency contact, a section the government official completes, a parental consent for a volunteer under 18, an affirmation with signatures, and a termination block.
The burden statement estimates an average of 15 minutes per response. You can fill it out free in your browser on this page and download the result without an account.

Individual or group, group name, name of the volunteer or group leader, U.S. citizen or permanent resident (with visa type), address, date of birth, phone and email address.
Ethnicity, race (one or more), whether you are a veteran or on active duty, and whether you have a disability. The form says this information helps understand the diversity and inclusion of the volunteer force.
Name, phone, email address, street address, city, state and ZIP code of an emergency contact.
Agency or bureau, agreement number, agency contact, whether reimbursements are approved with type and rate, position or project title, a description of the service, and checkboxes for attachments and requirements.
Contact details for the parent or guardian of a volunteer under age 18, the name of the youth, and the parent or guardian signature and date.
The volunteer and group leader affirmation boxes, signatures of the volunteer or group leader and of a government representative, then the termination date, total hours completed and a government representative signature.
The form has 46 numbered items over two pages, all in Chinese.
Check individual or group in item 1 (choose 1). For a group, enter the group name in item 2.
Complete items 3 to 19. Item 12, demographic information, is marked optional.
The government official completes items 20 to 27. The form says the service description should include details such as time and schedule commitment, use of a government vehicle, personal equipment or vehicle, skills required and physical activity level.
For a volunteer under age 18, a parent or guardian fills in items 28 to 36, signs item 37 and dates item 38.
Check the boxes in item 39 and enter the name of the federal agency. The volunteer or group leader signs item 40 and dates item 41, and a government representative signs item 42 and dates item 43.
When the agreement ends, items 44 to 46 record the termination date, total hours completed and a government representative signature.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address. The agency named in item 20 completes its own section, and a government representative signs for the agency.
The form says the agency agrees, while the arrangement is in effect, to provide the materials, equipment and facilities needed for the service, and to treat the volunteer as a federal employee only for tort claims, liability and injury compensation where the volunteer group does not cover them.
The affirmation says either the government or the volunteer may cancel the agreement at any time by notifying the other party.
The burden statement estimates an average of 15 minutes per response, including time to review the instructions and gather the data.
Collection and use are covered by Privacy Act System of Records INTERIOR/DOI-05 and OPM/GOVT-1, under 5 USC 552a. The form says completing it is voluntary, but not providing the information will prevent participation in the program.
