Request for Information Processing or Data Communications and Networking Support
GSA Form 917, Request for Information Processing or Data Communications and Networking Support, is a General Services Administration form, GSA FORM 917 (REV. 8-89). GSA hosts it in its forms library.
The form has two sections. Section I, Requester Information, is completed by the requesting GSA service or staff office: what support is needed, who to contact, the Privacy Act status of the work, and approvals. Section II, Support Office Information, records the support office's control number, target date and estimated cost, and the instructions on the back say the requester should not use it.
The back gives block-by-block instructions for Section I. They say requests must be approved by the Heads of Services and Staff Offices, Regional Administrators, and their designated representatives, and that a completed GSA Form 3537 must be attached or action cannot be taken. You can fill it out free in your browser on this page and download the result without an account.

Descriptive title, requester reference number, requesting organization, correspondence symbol, ADP user code or GSA appropriation number, and the liaison contact with building, room and telephone.
The requester's signature and date, an effective date, the service or staff office approval, and the type of support: new recurring, one-time special, modification to existing system, or other.
Three choices: the services do not include processing of a system of records; include one whose notice was published in the Federal Register on a given date; or include a new one to be published not later than 30 days prior to implementation.
The instructions give an outline: background, purpose, action required, schedule, security classification and cost statement. Extra sheets may be attached.
The plan activity number, whether GSA Form 3537 is attached, and whether a cost study is required before initiation.

Control number, target date, support office, estimated development and recurring cost in dollars, and the signature and date of the support office director.
The instructions say the descriptive title must not exceed 125 characters. The reference number uses a two position correspondence symbol, a three digit 917 number and a two digit fiscal year, as in AA-001-85.
For block 5, the instructions say that when the support requires automated information acquisition, the appropriation or fund must be provided.
When block 8 has a date, the instructions say block 12 explains what will happen if the date is not met.
Block 13 cites the activity number from the GSA 5-Year Strategic Plan for Automated Information. The instructions say support not in the Plan requires the Plan to be modified first.
Check block 14 and attach a completed GSA Form 3537. In block 15, Yes requests a statement of costs, and the instructions say support is then not provided until the requesting office approves the estimate.
The requester signs block 7, the service or staff office approves in block 9, and Section II is left for the support office.
These steps follow the instructions printed on the back of the form.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no address. Section II is completed by the support office, and its director signs the review in Section II, block 5.
The instructions cite the Privacy Act of 1974, 88 Stat. 1895, 5 U.S.C. 552a, and point to GSA Order ADM 7900.4 for definitions and procedures.
The block 12 outline asks for any national security or other security requirements, and lets the requesting office ask for a periodic estimate of accumulated project costs.
The footer reads GSA FORM 917 (REV. 8-89). GSA's page lists a current revision date of 08/1989 and no authority or regulation.

Block 3 asks for the full title of the requesting office, the service or staff office making the request, and block 4 for its correspondence symbol.
Block 7 is signed and dated by the person who has the authority to request information processing or data communications and networking support.
Block 9 is the service or staff office approval. The instructions name the Heads of Services and Staff Offices, Regional Administrators, and their designated representatives.
Block 6 names the person who can make technical decisions related to the request, with building and room number and telephone.
Section II is for the support office: control number, target date, support office, estimated development and recurring cost, and the support office director's review. The requester should not use it.
