Motor Vehicle Operator's License and Driving Record
GSA Form 3607, Motor Vehicle Operator's License and Driving Record, is a two-page GSA form. The footer reads GENERAL SERVICES ADMINISTRATION and GSA 3607 (REV. 3/2025), and GSA's page lists OAS 5620.1 as its authority. GSA hosts it in its forms library.
The Privacy Act statement on the back says the principal purposes are to provide necessary data to determine whether the applicant is competent to operate a federal motor vehicle, and to provide a written record of the applicant's previous driving record, physical fitness, and ability. The applicant gives details from a current driver's license and a driving record for the past four years, then signs a certification. A supervisor reviews it, and a certifying official signs the certification of eligibility and authorization for vehicle Types A to E.
You can fill it out free in your browser on this page and download the result without an account.
