Monthly Home-to-Work Transportation Log
GSA Form 3601, Monthly Home-to-Work Transportation Log, is a two-page GSA form. The footer reads GENERAL SERVICES ADMINISTRATION and GSA FORM 3601 (10-88), and GSA hosts it in its forms library.
The instructions on the back cite 41 CFR PART 101-6 and say employees who are provided with home-to-work authorization are required by law to maintain a record of such trips, and that GSA has determined that a monthly log be used. The front records the employee and the authorization, then one line for each day transportation is provided: the circumstances, field work or other than field work, the vehicle license tag number, and a round trip or one way trip. Totals are computed at $3.00 per round trip and $1.50 per one way trip. The employee and the controlling official sign each page. A Privacy Act statement is printed on the back.
You can fill it out free in your browser on this page and download the result without an account.
