GSA Form 87: Official Travel Authorization
GSA Form 87, Official Travel Authorization, is a General Services Administration form, GSA 87 (REV. 1/2020), hosted in GSA's forms library. The Privacy Act statement on the back says the principal purpose of the data is to determine the amount to reimburse an employee for expenses incurred in connection with temporary duty travel.
The front has 25 numbered blocks: the authorization number, the traveler's name, title, Pegasys vendor code, residence address, office and duty station, the travel purpose and reason, the transportation authorized, the official itinerary with per diem rates, yes or no questions on personal deviations, contract carriers, other-than-coach travel, actual expense and the government travel charge card, the estimated cost, any travel advance, remarks, the Pegasys account class, and two signatures.
The back carries block-by-block instructions. The traveler or travel arranger (invitational travel only) signs block 24, and the travel authorizing official signs block 25. You can fill it out free in your browser on this page and download the result without an account.
