Packaging, Packing and Marking Deficiency Report
GSA Form 3279, Packaging, Packing and Marking Deficiency Report, is a two-page form: the report on the front and instructions on the back. The footer reads GENERAL SERVICES ADMINISTRATION and GSA FORM 3279 (REV. 9-84), and GSA hosts it in its forms library.
Part A is completed by supply depot personnel. It opens with the printed statement that receipts of the subject items present excessive facility operating costs and that the following actions are recommended, then records the item, the contractor and manufacturer, the shipping container, the inner package, a description of the problem and recommended changes. Part B is completed by the reviewing office, which records the action taken, with signature lines for packaging, quality assurance and contract administration. The form prints no OMB number and no privacy statement.
You can fill it out free in your browser on this page and download the result without an account.
