Quality Deficiency Notice
GSA Form 3539, Quality Deficiency Notice, is a General Services Administration form, GSA FORM 3539 (11-84). GSA hosts it in its forms library; GSA's page lists no authority or regulation.
It is a one-page notice addressed to a supplier. It names the supplier and prime contractor, the contract, order and lot numbers and the NSN, and describes the nature of a deficiency in the supplier's quality control system. The form says immediate action is required to correct the deficiency and the condition that caused it, and asks for a written response of correction action taken or to be taken to your OAS within 5 workdays after receiving this notice.
The lower half records the evaluation of the correction action. If the action is not acceptable or not implemented, the form says the matter is being referred to the Administrative Contracting Officer. You can fill it out free in your browser on this page and download the result without an account.
