Quality Review
GSA Form 2522, Quality Review, is a General Services Administration form with instructions on its second page. The footer reads GSA FORM 2522 (REV. 7-94) and Prescribed by FSS P 2901.5, and GSA hosts it in its forms library.
The form records a review of a supplier taking part in Source Inspection by Quality Approved Manufacturer (SIQAM). The instructions say to audit and evaluate the adequacy and compliance of the system by observing and reviewing the contract requirements, technical documents, and the supplier's documented quality system and related process control records. It then records a check of inspection and test results for a product, any unsatisfactory conditions and the corrective action to be taken.
The QAS or LQAS signs item 13, and the SQAS or a delegated LQAS signs item 14 after review. The form does not spell out these titles. You can fill it out free in your browser on this page and download the result without an account.

The supplier, company officials contacted, any prime contractor, and the contract numbers and specifications reviewed.
Yes or no questions on completing and distributing SIQAM certificates, notifying the ACO, following written inspection and test procedures, keeping inspection records, segregating rejected materials and calibrating equipment.
Whether test procedures reflect the specification revisions, changes since the last review, verification of earlier corrective actions, whether the system meets Federal Standard 368, and the date of the last complete review.

Explanations of NO answers to questions 5, 6, 7B, 7C, 8, 9A and 9B, and of a YES answer to question 7A.

Nomenclature, national stock number, order numbers and quantity, then compliance (C) or noncompliance (NC) for product inspection, tests and preparation for delivery, the verification method and whether findings are attached.
Unsatisfactory conditions, corrective action to be taken (with the QDN number and date if one is issued), and the QAS and supervisor signatures.
Enter the name, address and ZIP code of the SIQAM supplier in item 1. The instructions call items 2 to 4 self explanatory.
Check yes, no or annotate nonapplicable (N/A) as appropriate, and enter the date of the last complete review in subitem 9C.
Explain any NO answer to questions 5, 6, 7B, 7C, 8, 9A and 9B and a YES answer to question 7A.
The instructions say item 11 entries refer to the product being audited at the time of review: the item name, national stock numbers, purchase order numbers and total quantity, then compliance or noncompliance and an X for the verification method.
Check yes or no for attached manufacturer's findings and supporting documentation, attach them, explain unsatisfactory conditions in 12A, and give the supplier's corrective actions in 12B.
The QAS/LQAS signs or identifies and dates item 13. After review, the SQAS or delegated LQAS signs and dates item 14.
The instructions on page 2 cover each item.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The form prints no address. It ends with the QAS signature and the supervisor's review in items 13 and 14.
The form says to continue on a separate sheet, referencing the item numbers continued.
The item 13 instruction allows the QAS/LQAS to individually identify, for example by identification number, for electronic form transmissions instead of signing.
The footer reads GSA FORM 2522 (REV. 7-94) and Prescribed by FSS P 2901.5. GSA's page lists a current revision date of 07/1994 and FSS P 2901.5.
