Contact Record
GSA Form 2429, Contact Record, is a General Services Administration form. Its footer reads GSA FORM 2429 (REV. 8-90), and GSA hosts it in its forms library. GSA's page lists ADM 5400.7 as its authority or regulation.
The form's instruction line says: Summarize all contacts involving official business, to include minor matters that do not merit a memorandum. (Complete by hand). The one-page record takes a case file number, the commodity, whether the contact was in person or by telephone and incoming or outgoing, the telephone number called, the date, time and a signature, the persons contacted and their organization, and a routing table. Three numbered blocks hold a summary of discussion, commitments made and target dates, and remarks. The PDF's second page is the blank reverse for text continued from those blocks. The form prints no OMB number and no privacy statement.
You can fill it out free in your browser on this page and download the result without an account.
