Optional Form 41 (OF 41): Routing and Transmittal Slip
Optional Form 41 (OF 41), Routing and Transmittal Slip, is a one-page slip printed as OPTIONAL FORM 41 (REV. 5/2020). The form names no issuing agency; GSA hosts it in its forms library.

Write each recipient on a numbered TO line with name, office symbol, room number, building and agency or post. There are eight lines.

Mark the box for what the recipients are asked to do, such as Approval, Comment, Prepare Reply or See Me.

Write any note in Remarks, then complete the FROM block with the sender's name, organization symbol, agency or post, room number, building and phone number.
