Business Service Center Activity Report
GSA Form 1919, Business Service Center Activity Report, is a two-page work unit report. The footer reads GENERAL SERVICES ADMINISTRATION and GSA FORM 1919 (REV. 4-87), and GSA hosts it in its forms library. GSA's page lists a current revision date of 04/1987 and no authority or regulation.
The header has boxes for the region and the month and FY, and prints RCS 0A-42. The front counts work in five categories: business counseling, contract support services, new items, preferential contracting programs, and public information and special projects. Counseling actions are split between small business (minority and non-minority, male and female) and large business, and every row has columns headed FSS, FPRS, IRM, PBS and General. Item 6 on the back is for supplemental information, followed by the instructions.
The form does not name who prepares or receives it, and it prints no signature line, OMB number or privacy statement. You can fill it out free in your browser on this page and download the result without an account.
