GSA2689
Small Business Analysis Record
GSA Form 2689, Small Business Analysis Record, is a General Services Administration form. Its footer reads GSA 2689 (REV. 6/2026), and GSA hosts it in its forms library, where its page lists the authority or regulation APD 2800.12B.
Page 1 describes an acquisition: submission type, contracting office details, funding, product and service code, NAICS code and size standard, estimated dollar values, period of performance, a description, any prior acquisitions, the acquisition strategy, market research, contract type and FAR questions, then the contracting officer's electronic signature. Page 2 holds small business participation recommendations, concurrence and electronic signatures from the OSDBU Small Business Technical Advisor, the Associate Administrator OSDBU and the SBA Procurement Center Representative. Page 3 gives field-by-field instructions, which say all FAR references are to deviated language applicable to GSA's acquisition system pursuant to the Revolutionary FAR Overhaul.
