Register of Top Secret Material
GSA Form 1567, Register of Top Secret Material, is a General Services Administration form, Rev. 9-74. GSA hosts it in its forms library.
The one-page register is a table. The organizational unit's name and location go at the top, and each row records a control number, a copy number, the person originating or receiving TS material, the date, the title or identification of the material and the number of copies.
The form prints no instructions, signature line, filing address or OMB number, and it does not say who keeps the register. It is an internal record for classified material, not a form the public files. You can fill it out free in your browser on this page and download the result without an account.

Name and location of the unit, in the top right box.
The first two columns are headed Control No. and Copy No.
The person originating or receiving TS material, and the date.
The title or identification of the material.
The last column records the number of copies.
The header and six columns repeat on every row.
Enter the organizational unit's name and location at the top.
Start each row with the control number and the copy number.
Enter the person originating or receiving the TS material and the date.
Add the title or identification of the material and the number of copies.
The form gives no written steps. This follows the order of its columns.
You get the real form, filled in a browser tab, with nothing standing between you and the finished PDF.
The PDF gives no filing address or routing. The only office it names is the organizational unit in the header.
The footer reads General Services Administration, GSA Form 1567 (Rev. 9-74). GSA's page lists the current revision date as 09/1974.
The form prints no instructions, OMB number or Privacy Act statement.
GSA also hosts GSA Form 1566, Top Secret Control Sheet, which uses the same control number and copy number fields. Form 1567 does not name it.
