Recommendation for Award
GSA Form 1535, Recommendation for Award(s), is a two-page General Services Administration (GSA) form, printed as GSA FORM 1535 (REV. 3-89). GSA hosts it in its forms library, and GSA's page lists APD 2800.12B as its authority or regulation.
The front records a solicitation and the awards proposed under it: solicitation number, issue and opening dates, contract period, supplies or service, total dollar value, contract type, procurement method, number of offers, award status and to whom the items are awarded. Block 12 lists each proposed award with acquisition coding, contractor, item numbers, production point and dollar amount. It is signed by the preparer, recommended by a supervisor and approved by a contracting officer, with concurrences as applicable.
The back is a 27-line checklist with FAR, GSAR and APD citations and a Remarks space. You can fill it out free in your browser on this page and download the result without an account.
