GSA1560
Recommendation of Evaluation Panel
GSA Form 1560, Recommendation of Evaluation Panel, is a General Services Administration form, Rev. 8/1991. GSA hosts it in its forms library.
The form records the finding of an evaluation panel established under a GSA Order. The panel members attest that they examined the evidence of a trainee's progress in relation to his/her Individual Development Plan, and check whether the trainee has achieved promotion to the next appropriate grade or reassignment to the target position. A Training Officer then verifies that the training program requirements were met.
The instructions say the panel forwards the completed form to the Training Officer, and that for promotion or target position assignment a Standard Form 52 (Request for Personnel Action) should accompany it. You can fill it out free in your browser on this page and download the result without an account.
