Authorization, Agreement, and Certification of Training
Standard Form 182 (SF 182), Authorization, Agreement, and Certification of Training, is a U.S. Office of Personnel Management (OPM) form. GSA hosts a copy in its forms library. The privacy statement says the information is collected under 5 U.S.C. 4115, a provision of the Government Employees Training Act, and that its main purpose is to document the approval and completion of employee training within an agency.
The form has six sections: trainee information, training course data, costs and billing, supervisor approvals, the authorizing official's approval, and the certification of training completion and evaluation. Pages 4 to 9 explain the codes for fields such as position level, education level, training purpose, training type, delivery type and training source.
The form says completed training information is provided electronically to a Governmentwide system, where it becomes part of the permanent employment record. You can fill it out free in your browser on this page and download the result without an account.
